The practitioners at Olympic Park Sports Medicine Centre use shared patient files and therefore all patient records collected at this clinic will be available to any practitioner you see at this clinic.

Should you wish to attain your medical file, you will need to put a request in writing stating the following: your full name, address, date of birth and your signature. Please note we have 45 days to respond to your request. There may be a fee charged.
